Get details on restaurants cited by Maricopa County inspectors for four or more priority violations the week of Sept. 18. 

A priority violation is a major violation that directly contributes to increasing the risk of food-borne illness or injury. If listed in the reports, remedies implemented during inspections are included.

At bottom, see a sampling of the many restaurants graded as an A for no violations.

American Legion No. 41, 715 S. 2nd Ave., Phoenix.

  1. Employee handled limes with bare hands. Lime discarded. Employee used tongs.
  2. Chicken tacos, rice, beans and pastries made at home and served in bar.
  3. Raw shell eggs stored above tortillas. Items rearranged.
  4. Chlorine sanitizer concentration at zero. Concentration restored to proper level.

Original Cuisine, 1853 W. Broadway Road, Mesa.

  1. Raw pork stored above bean sprouts, cauliflower and celery. Items rearranged.
  2. Cooked bean sprouts and hot chili mix held below proper temperature. Reheated.
  3. Raw shell eggs, cooked garlic and cooked beef and vegetables held at improper temperature. Eggs moved to cooler, garlic and cooked beef discarded.
  4. Food stored in direct contact with “to-go” bags. Food moved to proper food storage containers.

Blue 42 Sports Grill, 4845 S. Arizona Ave., Chandler.

  1. Chlorine sanitizer concentration at zero. Concentration restored to proper level.
  2. Cooked peppers and cooked chicken expired. Discarded.
  3. Inside of copper cups turning green. Cups removed from bar.
  4. Leak from drain line in sink. Must repair prior to re-inspection.

Four Seasons Sushi Bar and Grill, 1652 N. Higley Road, Suite A-101, Gilbert.

  1. Facility manager unable to provide the parasite destruction document for seafood restaurants. 
  2. Chlorine sanitizer concentration in dishwasher at zero. 
  3. Utensils and knives found with food debris. Utensils and knives washed and sanitized.
  4. Cooked shrimp, seafood salad, beef, raw fish and cut lettuce held at improper temperature. Discarded.
