Here are the details on restaurants Maricopa County inspectors cited for four or more priority violations during the week of Aug. 17. 

A priority violation is a major violation that contributes to increasing the risk of foodborne illness or injury. If listed in the reports, remedies implemented during inspections are included.

At the bottom, see a sampling of the restaurants graded as an A for no violations.

6 violations

Empire China Bistro, 7641 E. Guadalupe Rd, Suite 105, Mesa.

  1. Restaurant used an herb in its sauce brought over from China by a family member, rather than an approved food source. | Removed ingredient. 
  2. Noodles on top section of cooler growing mold. | Discarded. 
  3. No chemical sanitizer running through the dishwasher machine.  
  4. Pot with meat left on counter over night at 87 degrees. | Discarded. 
  5. Solution in bucket of bleach too high, considered a toxic substance. | Solution remixed. 
  6. Broken in-reach cooler along the cooking line, eggs and tofu brought to unsanitary temperatures. | Ingredients discarded. 

Pho Lotus Sai Gon Boyz Sandwiches, 835 N. Gilbert Rd., Gilbert.

  1. Employees handling food with bare hands. | Staff washed hands and put food handler gloves on. 
  2. Knives on holding racks soiled with accumulation of organic debris. | Utensils removed to be scrubbed and sanitized. 
  3. Raw meats were placed over noodles and vegetables. | Relocated to proper place. 
  4. Cooked chicken and pork pans left on counter at too high of a temperature. | Discarded. 
  5. Cooked foods placed in food cooler at temperature danger zones. | Discarded. 
  6. Several ingredients observed to be past discard date. | Discarded.

Rustler’s Rooste, 8383 S. 48th St., Phoenix.

  1. Employee put on gloves to handle food without washing hands. | Manager had employee wash hands. 
  2. Employee handled ready to eat garnish with bare hands. | Discarded. 
  3. Cooked potato skins held in chafer was not under a time control. | Redirected to be placed a until time control. 
  4. No approved back-flow prevention along water stream of mop sink faucet. 

Mariscos Las Islitas, 35th Ave./Van Buren @ 3, Phoenix

  1. Employee wash hands for approximately 10 seconds. | Re-washed for longer time. 
  2. Raw shrimp placed above ready to eat food. | Rearranged. 
  3. Cooked meat placed in too hot of a holding unit. | Owner moved beef. 
  4. Bags of vegetables and shrimp placed in too cold of a holding unit for an extended period of time. | Discarded.
