Get details on restaurants cited by Maricopa County inspectors for four or more priority violations the week of Dec. 10.

A priority violation is a major violation that directly contributes to increasing the risk of food-borne illness or injury. If listed in the reports, remedies implemented during inspections are included.

At bottom, see a sampling of the many restaurants graded as an A for no violations.

Shaanxi Garden, 67 N. Dobson Road, Unit 109, Mesa.

  1. Employee washed dirty dishes and then returned to peeling and handling potatoes without washing hands. | Hands washed. Repeat violation: Third consecutive violation.
  2. Raw fish stored above cilantro, raw eggs stored above lemons, and raw chicken stored above ready-to-eat sauces in walk-in refrigerator. | Items rearranged to prevent cross contamination. Repeat violation: Third consecutive violation.
  3. Food debris on can opener, dicer and meat grinder, deli slicer, bread maker and roller/noodle maker. Some of the items are not approved for food contact surfaces. | Can opener, dicer and meat grinder were washed, rinsed and sanitized.   1. Raw beef, raw chicken, raw pork and fish balls were stored, prepared and/or handled at a private residence. | Items discarded. Repeat violation: Second consecutive violation.
  2. Raw chicken and rabbit stored above cooked lamb soup and jelly, raw fish stored over pickled radish and raw pooled eggs stored over pickled radish in cooler. | Rearranged to prevent cross-contamination. Repeat violation: Second consecutive violation.
  3. Not enough chlorine sanitizer in sink. | Remade.
  4. Build up of food debris on deli slicer and several peelers. | All items washed, rinsed, and sanitized. Repeat violation: Second consecutive violation.
  5. Cooked soup below proper temperature. | Discarded.
  6. Raw shell eggs stored at elevated temperatures. Various food items, including cooked beef, cooked crawfish, cooked noodles, cut lettuce, stored in walk-in refrigerator above proper temperatures. | Corrected. Repeat violation: Fourth consecutive violation.
  7. Multiple cans of liquid ethanol stored above dry chiles and dry herbs in dry storage area. Bottle of glue stored over food-prep area. | Chemicals moved.

  1. Box of moldy cucumbers stored in cooler. Moldy cooked pork chops found in a case with deli meat and cheese. | Items discarded.
  2. Box of oranges stored in a metal pan with raw marinating chicken in cooler. | Items moved.
  3. Food debris on knives and dicer. | Items were put in dishwashing area to be cleaned.
  4. Cooked beef, pork, rice and vegetables at improper temperatures. | Items discarded.
  5. Cooked onions and cooked peppers at improper temperatures. | Items reheated.

Doc & Eddy’s, 909 E Minton Drive, Tempe.

  1. Dishwasher failed to dispense chlorine sanitizer. Not enough ammonium concentration in three-compartment sink. | Corrected.
  2. Au jus at improper temperatures. | Discarded.
  3. Bottles of mixers stored inside ice bin. |Bottles removed. Ice discarded.
  4. Backflow-prevention device missing from hose connected to an exterior faucet. | Hose disconnected.

Red Dragon, 2160 N. Alma School Road, Suite 114, Chandler.

  1. Employee cracked eggs into wok then proceeded over to the walk-in cooler, dropped hat on floor, picked it up and scooped rice without washing hands between tasks. | Hands washed.
  2. Raw beef intermingled with raw chicken breasts during thawing. | Rearranged to prevent cross-contamination.
  3. Cheese mix stored at improper temperature. |Discarded.
  4. Cooked yellow rice at elevated temperature. | Moved to cooler.

Fat Willy’s Family Sports Grill, 2250 S. Buttercup, Mesa.

  1. Food debris on meat slicer and can opener blade from previous dayâ??s use. | Items cleaned.
  2. Several food items, including salsa, salad dressings, cocktail and tartar sauces found in shallow pan with melted ice at elevated temperatures. | Discarded.
  3. Cooked chicken wings in cooler past discard date. | Discarded.
  4. Leaky atmospheric vacuum breaker connected to mop sink spigot.

Akita Sushi, 9011 E. Via Linda, Scottsdale.

  1. Employee handled soiled dishes, rinsed hands with sprayer, wiped hands on soiled towel and apron then proceeded to rack clean dishes without washing hands between tasks. | Washed hands. Repeat violation: Third consecutive violation.
  2. Raw jalapenos and Chinese eggplant in cooler growing mold. | Discarded.
  3. Various types of raw fish stored above cooked octopus, crab mix and avocado in prep coolers at sushi station. Raw salmon and spicy tuna stored above ready-to-eat cabbage, cooked crab mix and avocado. Raw beef stored above beer in cooler. | Rearranged to prevent cross-contamination.
  4. No chlorine sanitizer during dishwasherâ??s final rinse. | Changed empty sanitizer bucket and rewashed dishes.  Repeat violation: Third consecutive violation.

Qwik Chinese Bistro, 9030 E. Via Linda, Scottsdale.

  1. Wide-mouthed containers with mini spoon and lid used to serve chili and sweet and sour sauce to customers. | Containers pulled from station to prevent cross-contamination.
  2. Several food items, including raw beef, raw chicken, raw fish, cooked rice, cooked chicken, raw shelled eggs, tofu and cream cheese found in walk-in cooler at improper temperatures. | Cooked rice and tofu discarded. Other items moved to another cooler. Repeat violation: Third consecutive violation.
  3. Floor sink beneath prep cooler and mop sink drain filled almost to inlet with water. Three-compartment floor sink, full to inlet, overflowed onto floor when faucet turned on. | Full strainers cleared. Plumbing systems to be repaired.
